Home Sleep Tests (HST) should be utilized only as part of a Comprehensive Sleep Evaluation. You can print off and fill out the Epworth Sleepiness Scale and the Berlin Questionnaire and take them to your physician along with the order form. The referring provider can order the test if they determine that the patient meets the criteria for HST.
